To make an appointment, please call (618-288-9450). When calling for an appointment, please be prepared to provide the patient’s name, address, phone number, birth date, type of insurance, reason for appointment, and referring physician, if applicable. We see patients by appointment only.

8:00 a.m. - noon and 1:00 p.m. - 5:00 p.m. Monday - Thursday and
8:00 - noon on Friday.

Directions:
The practice is located on IL-159 between Rte 162 and I-270.
From I-55: Get off at exit for Maryville/Collinsville, Route 159. Go North on Rte 159 towards Edwardsville. Office will be on the right between Route 162 and I-270. (Just past Field Crossings at top of the hill).
From downtown Edwardsville/ I-270: Get off at Edwardsville/Collinsville exit, Rte 159. Drive South on Rte 159 towards Collinsville. Go through Glen Crossing light. At the rise of the hill, office is on the left.
